Program Registration Form

This registration form is to be used for all Recreation Department Programs: Please do not use this form for Tc3, Summer Skies, Kiwanis Baseball or Village Camps as we do not manage these programs.

Volunteer Application

All department coaches and volunteers must fill out this form and return it for approval prior to volunteering at any Town of Dryden Recreation program or event.

Scholarship Application Form  

For participants seeking financial assistance. This form allows the Recreation Department to verify eligibility for SNAP received through school, so children who receive free or reduced lunches at school also receive free or reduced pricing on programs.

Adult Softball Roster Form 

Download and print this form to be completed by team members and send to the Recreation Department office at 93 East Main Street, Dryden, NY 13053. You may also scan a completed form to be sent to recassist@dryden.ny.us

Official Report Form

If you have an issue with a program coach, volunteer, or department staff member, please print this form and submit it to the Recreation Director. We encourage people to discuss the concern with the volunteer, coach, or staff member before submitting this form.

Program Proposal Form 

If you have an idea for new activities that you would like to submit to the Recreation Department, please print off this form and return it completed to our office. Not all program proposals will be run, but we will examine all proposals for viability.
